Federal law requires MedConnect Pro to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of our legal duties and privacy practices. We are required to follow the terms of this Notice currently in effect.

Breach Notification
In the event of a breach of your unsecured PHI, we will notify you as required by law. We will inform you of the breach, what information was involved, steps we are taking, and what you can do to protect yourself.
Changes to This Notice
We reserve the right to change this Notice. Any changes will apply to health information we already have as well as information we receive in the future. We will post the current Notice in our office and on our website, and will provide you with a copy upon request.

If you believe your privacy rights have been violated, you may file a complaint with:

You will not be penalized or retaliated against for filing a complaint.
